Error and Risk Analysis

The Event and Risk Communication Analysis (ERiKA) retrospectively identifies all systematic causes which have led to an adverse event during the treatment of a patient.

Analyses of adverse events provide information on all factors that endanger patient safety in clinical practice. As a result, specific prevention measures for minimizing and eliminating risk potential can be derived.

We recommend the use of ERiKA following

  • occurrence of severe or moderate injuries to patients as a result of medical errors 
    (e.g. wrong-side surgeries) 
  • near misses (e.g. timely discovery of infusion confusion)
  • allegations of medical malpractice or patients' complaints 

We recommend to carry out the Event and Risk Communication Analysis (ERiKA) as soon as possible to prevent any repeated occurrence.
